Our Behavioral Health services are at the forefront of our holistic mission to restore individuals and families to psychological, social, physical and spiritual health. Agape Network has been awarded a CCBHC-PDI SAMHSA grant to assist in the planning, development and implementation process to become a Certified Community Behavioral Health Clinic model. We provide fully coordinated and integrated care that addresses the multifaceted health issues experienced by our diverse community in Miami-Dade County. With a whole-person care model, our facilities offer residential inpatient and outpatient services, along with onsite locations across the county, ensuring accessibility and comprehensive support.
Our dedicated team of over 200 staff members is committed to providing personalized care plans and evidence-based interventions, utilizing population health technology to coordinate care transitions, identify high-risk patients and close care gaps. Through proactive predictive analytics and ongoing communication, we empower our clients to overcome challenges, achieve wellness and reach their full potential, in alignment with our vision of holistic well-being for all.
Our Primary Care services are an integral part of our commitment to whole-person care. With a six-acre, 70,000 sq. ft. community health center and additional facilities across the county, we provide accessible and comprehensive care by collaborating with community. Through our partnership with Health Choice Network, we leverage a national model of care by collaborating with community health partners to ensure effectiveness, cost-efficiency and improved patient outcomes.
Our population health technology enables us to assess patient needs, develop personalized care plans and monitor health remotely through telehealth/telemedicine, reducing nonadherence to treatment plans and avoidable hospital visits.
